Coroners’ Prevention of Future Deaths reports

When an inquest reveals a risk that more people could die, the coroner must send a Prevention of Future Deaths report to the organisations able to act, and they must respond within 56 days. These are the 275 reports published by the Courts and Tribunals Judiciary that were sent to, or name, the 11 organisations in this report.

Names of the people who died are not shown; every entry links to the official report on judiciary.uk. “No response published” means no reply from that organisation appears on judiciary.uk more than 56 days after the report; a reply may exist but not have been published, and older reports are more often missing replies. Gathered October 2026. Counts are a minimum.
